Expression of Interest are invited for Provision of the Health Services (Healthcare Professionals) to SMVAF for a Period of Three (3) Years. ADVERT SINCEPHETELO MOTOR VEHICLE ACCIDENTS FUND REQUEST FOR EXPRESSION OF INTEREST TENDER NO.: SMVAF/004/2021/22 The Sincephetelo Motor Vehicle Accidents Fund (SMVAF) is a Category A enterprise created as an instrument through which the government of Eswatini treats, rehabilitates and compensates victims of road traffic accidents. It is a Fund also empowered to collaborate with stakeholders on the prevention of accidents. The Fund invites Healthcare Professionals wishing to be registered in the Fund-s database to express their interest for the provision of the health services to SMVAF for a period of three (3) years 1. General: Physicians Paediatricians General Practitioners 2.Surgeons: Orthopaedic Surgeon Neurosurgeons Maxillo-facial Surgeons Dentists Plastic Surgeons Cardiologist ENT Surgeon General Surgeons Urologist Ophthalmologists Gynaecologists Spinal Surgeon Neurologist Neuropsychiatrist Neuropsychologist Paediatrician Obstetrician Cardiologist 3.Support Services: Clinical Psychologist/Therapist Physiotherapists Dieticians Optometrists Occupational Therapists Industrial Psychologist Clinical Nutritionist and Dietician Educational Psychologist Podiatrist Speech Therapist Orthoptists Interested Healthcare Professionals should submit the following documents: 1) Medical and Dental Council Certificate 2) Proof of payment of tender document fee 3) Certified copies of Academic Qualifications and certifications 4) Brief Profile 5) A valid Tax Clearance Certificate 6) Certified Copies of National IDs or Passport 7) Police Clearance Certificates not older than 6 months 8) Copy of the audited financial Statements for the last two (2) years or the last 6 months certified bank statements. 9) Signed Declaration of Eligibility 10)A list of at least 3 traceable references The ‘Expression of Interest- document can be obtained at the reception of the Sincephetelo MVA Fund, as from Wednesday, 21st July 2021 for a non-refundable fee of E300.00 per set
